Required Forms
The following forms are designed to help us work together more easily. If you are unable to print the forms to bring to your first appointment, please be sure to arrive early enough that there is adequate time to complete the forms before your scheduled appointment time.
It will be asked for you to complete this form before any work is done. It includes a health history as well as liability release.
This form allows a parent or legal guardian to give permission to perform massage on a minor client.
General Liability Release Form
This is a standard release of liability for our massage therapy work together.
